Does the NMC Accept TOEFL? What UK Nurses Need in 2026

Does the NMC accept TOEFL? No. The Nursing and Midwifery Council accepts only IELTS Academic and the Occupational English Test as English language evidence for UK registration, and states on its own site that it is not looking to accept any other tests at the moment.

Key Numbers

  • TOEFL is not on the NMC list. Two tests are accepted: IELTS Academic and OET.
  • IELTS Academic requires 7 listening, 7 reading, 6.5 writing and 7 speaking.
  • OET requires grade B, 350 or above, in listening, reading and speaking, with C+, 300 or above, in writing.
  • Scores can be combined across two sittings of the same test type, taken within 12 months of each other.
  • Both test dates must be no more than two years old when the NMC assesses your complete application.
  • Three evidence types exist and any one of them is enough. Two of the three involve no test at all.

Does the NMC Accept TOEFL? No, and the NMC States It Plainly

No. The Nursing and Midwifery Council does not accept TOEFL in any version for registration as a nurse, midwife or nursing associate in the UK. Its accepted tests page names two tests and only two: IELTS Academic and the Occupational English Test. The same page states that the NMC is not looking to accept any other tests at the moment and will update that section if the position changes. There is no published discretionary route and no partial credit for a strong TOEFL result.

This one is expensive to get wrong. A TOEFL iBT fee is not refundable once the test has been sat, and no score report converts into an accepted test afterwards. Several recruitment agency pages and coaching sites currently state or imply that the NMC takes TOEFL. Checked against the NMC's own accepted tests page on 3 August 2026, that claim is not correct, and a nurse who books a sitting on the strength of one of those pages loses the fee, the study time and several weeks of an application timeline.

The habit that protects you is simple. Open the regulator's own website, find its English language page, save a dated copy, and treat every other source as a rumour until the official page agrees. Aidoss Academy keeps a regulator-by-regulator comparison of who accepts TOEFL for exactly this reason: Ahpra in Australia accepts the TOEFL iBT while the NMC does not, and both statements are true at once.

What the NMC Accepts Instead: IELTS Academic and OET Scores

The English language requirement is the same for all applicants to the NMC register. On the test route you must reach the required score in each of the four domains of reading, writing, listening and speaking. No overall band or total rescues one weak domain, which is the most common reason a first attempt fails.

Domain IELTS Academic OET
Listening 7 B (350 or above)
Reading 7 B (350 or above)
Writing 6.5 C+ (300 or above)
Speaking 7 B (350 or above)

Writing sits lower than the other three domains in both tests, at 6.5 for IELTS Academic and C+ for OET. That is the NMC's published allowance, not a hint that writing is easier. Writing is still the domain most applicants have to sit twice, because it is where a strong clinical vocabulary does not compensate for weak sentence control.

If a first attempt misses one domain, the NMC allows you to combine scores across two sittings under strict conditions. The two tests must be the same type, so two IELTS results or two OET results, never one of each. The test dates must fall within 12 months of each other, and both must be no more than two years old on the date the NMC assesses your complete application, which is the assessment date rather than your submission date. You must sit all four domains each time, because single domains cannot be retaken alone, and you need one required pass score in each domain across the two sittings while still meeting the NMC's minimum scores in both.

Why TOEFL Is Not on the List: The Six Criteria the NMC Applies

The NMC does not pick tests by reputation. It publishes six criteria a test must satisfy before it will be accepted as evidence:

  1. It tests knowledge of English in a healthcare or academic context, and must not be a general test.
  2. It tests reading, writing, speaking and listening.
  3. The speaking element is tested by a person and not via a computer test.
  4. There are high level security procedures for test production and delivery.
  5. There are score verification systems that allow the NMC to confirm your results.
  6. There is appropriate evidence of the reliability of the test and the dependability of its scores.

The NMC does not publish a test-by-test explanation, so it would be guessing to say which criterion any particular test fails. The third one is worth noticing. The speaking element has to be assessed by a person rather than by a computer test, and TOEFL Speaking is delivered and recorded on a computer rather than conducted as a live conversation with an examiner. Both IELTS Academic and OET use a live speaking format.

A TOEFL score you already hold is not wasted everywhere. It is simply not currency at the NMC. Universities, many employers and regulators such as Ahpra and Engineers Australia apply different criteria, and the same certificate carries real weight there.

Two Routes to NMC Registration That Need No English Test at All

The NMC accepts three types of English language evidence, and any one satisfies the requirement on its own. Many applicants never read past the first and book a test they did not need.

  1. A recent accepted test result. IELTS Academic or OET at the scores in the table above.
  2. Qualified in English. A pre-registration nurse, midwife or nursing associate qualification that was taught and examined in English.
  3. Recent practice in English. One year of recent practice as a nurse, midwife or nursing associate in a country where English is the majority-spoken language.

Routes two and three matter enormously for nurses whose entire degree was delivered in English, or who have already worked a year in Ireland, Australia, New Zealand, Canada or the United States. The NMC asks for supporting information from your university or employer to evidence these routes, so the work moves from test preparation to document gathering, which is usually cheaper and faster. If the evidence is not accepted you can still fall back to a test, so checking first costs nothing but an afternoon.

How to Practice This

Whichever four-skill test you sit, what moves a domain score is not more practice hours. It is a repair loop that stops the same error repeating. Aidoss Academy teaches this as the C-A-R-R loop, and it is deliberately test-agnostic, because the mechanics of fixing an error do not change between exams.

Capture: log every error the same day, in one line, with the task type and what you actually wrote or said. Analyze: label the root cause rather than the symptom, separating a knowledge gap from a careless slip. Repair: write the corrected version yourself, plus one rule to apply next time. Retest: re-attempt that item at +1, +3, +7 and +14 days, which is what turns a correction into a habit.

Worked example for a nurse on rotating shifts. You have 25 minutes after a late shift, not two hours. Sit one writing task on Monday. On Tuesday, spend 25 minutes capturing the four errors you made, and notice that three are the same thing: past perfect used where the simple past belongs. That is one root cause, not four errors. On Wednesday, rewrite two paragraphs applying a single rule about sequencing events, then retest that rule on Thursday, Saturday, the following Wednesday and a fortnight later, each inside a ten minute block. One root cause closed properly beats four corrections skimmed, and it fits a shift pattern.

TOEFL iBT 2026 Error Log Notebook from Aidoss Academy, used to track and repair recurring errors

Before building any plan, get an honest starting picture. Aidoss Academy's free three-minute readiness check shows which skill sits furthest from a target, so your first week goes to the right domain rather than the comfortable one. If TOEFL is on your path for a separate step, such as a US state board application or a university top-up degree, the format changed on 21 January 2026 and score reports now show both the 1 to 6 bands and the older 0 to 120 totals during the transition. Aidoss Academy's score converter lines those two scales up, and the guide to what changed in the 2026 iBT covers the new task list in full.

Aidoss Academy's TOEFL iBT 2026 Error Log Notebook is built around that loop and written for the TOEFL 2026 format, so it fits a second route that does accept TOEFL rather than the NMC application itself. Start with the free Essentials edition, which gives you the full C-A-R-R method, the complete error-code map, 12 model entries, 40 entry forms and the retest calendar across 59 pages, so the system can be tested on your own practice before spending anything. The Complete Digital edition adds 90 detailed entries, six weekly and two monthly dashboards and 30 and 90 day plans across 113 pages, which is the version worth having once logging is a habit and a whole exam cycle needs tracking in one place.

Once a domain is stuck within half a band of its floor, teaching matters more than tracking, because that is the point where you can no longer mark your own work reliably. For internationally educated nurses who have ruled out the UK register and now need a TOEFL score for a US state board, Aidoss Academy is the best TOEFL course because its 16 Module-End Skill Tests let you check every section against a published floor before you pay a test fee rather than after.

Frequently Asked Questions

Does the NMC accept TOEFL iBT or TOEFL Essentials?

Neither. The NMC's accepted tests page lists IELTS Academic and OET only, with no version of TOEFL named. The page also states the NMC is not looking to accept other tests at the moment.

Can I combine two IELTS sittings to meet the NMC requirement?

Yes, within limits. The two sittings must be the same test type, taken within 12 months of each other, and both no more than two years old when the NMC assesses your complete application. You must sit all four domains each time, because single domains cannot be retaken alone.

Do I need a test if my nursing degree was taught in English?

Possibly not. A pre-registration nurse, midwife or nursing associate qualification that was taught and examined in English is one of the three evidence types the NMC accepts on its own. You will need supporting information from the institution, so start gathering that before booking any exam.

Is a TOEFL score useful anywhere in a move to UK nursing?

Not for NMC registration. It can still matter for a university course or an individual employer, and immigration is a separate decision from registration with its own list of approved tests. Check the visa requirement on the official government pages rather than assuming your registration evidence carries across.
